We believe that our mission is to enrich lives. That begins with every life that we encounter: our employees, our patients and those who love and care for them, the communities in which we operate, our stakeholders, and those who are yet to become our customers.
Philosophy/Belief Statement:
We promise to care for the mind, body, and spirit in a way that is comforting and compassionate. We promise to be the difference in our customers' day...working to make every connection a personal one; and creating memorable moments that define their Heartland experience. We promise to discover what is important to our customers; respect their choices; and customize our service specifically to their needs. We promise to be leaders in care delivery, corporate responsibility, and community action.
Program History:
Heartland Hospice is part of the ProMedica family, the 15th largest healthcare system in the nation and the largest not-for-profit senior living organization for skilled nursing, assisted living, and hospice care. We deliver care in 30 states and are the third largest hospice provider in the nation. Our Fond du Lac office provides hospice care only. We service patients in Northeastern Wisconsin from the Oshkosh area to Kewaskum to Lake Michigan to Green Lake. We are surrounded by offices in Green Bay, Milwaukee, Stephen's Point, and Madison. This allows us to serve patients with a quicker response time.
Additional Information:
Can you spare a few hours each month to make a difference in the lives of terminally ill patients and their families in your town and the outlying communities?
Heartland Hospice of Fond du Lac invites caring, compassionate individuals to join our team. By sharing your time with someone during their final journey, you can bring companionship, support, joy, and laughter into the life of a person in need. This is also a great opportunity for students who are considering a career in the medical field to get some experience with the geriatric community.
As a Heartland volunteer, you can donate as much or as little of your time as you desire. The flexibility of our program allows you to enrich the lives of others based on your schedule and availability. All volunteers receive orientation and training to give them the skills they will need to make visits more meaningful. We also welcome volunteer with clerical and administrative skills who can help out in our office.
